One other thing I may be able to contribute is to the Varsity Sandy's in Minnesota. This store was located across the street from the campus of the University of Minnesota in Dinkytown (Dinkytown is the name given by the locals to a small business district located next to campus) and was a storefront location. It was not a separate building. It was open in the morning at 6:00 a.m. and I used to stop there for coffee and chocolate dipped donuts before I went to class. I don't have any pictures though - sorry.
